To join AFJRA ~ please print out application and fill out. Mail all necessary paperwork (application, birth certificate, report card schedule and $80 application fees ($30 insurance, $25 medical, and $25 family membership). Remember to get everything notarized before you mail out. If there are items missing the whole application will be returned. |
